- We the parents/legal guardians of the above named child, registering for the baseball programs governed by CBC Creeks Baseball Club, hereby give our consent for our child’s participation in all activities associated with said program. In consideration of registration, we agree to release and waive CBC Creeks Baseball Club, all agents, directors, officers, sponsors, volunteers, vendors and employees, of liability arising from personal injury, other than provisions covered under the parks maintained insurance policies, which may occur as a result of participation in the Parks activities. It is also our responsibility to communicate any limitations or illness that would restrict full participation in any activities.
